NEW CLIENT FORM

This form is for clients to fill out and sign after we have met. Welcome to All the Wag!

  • All the Wag Contract

  • TERMS AND CONDITIONS

    1-The term of this contract shall be for any and all services and dates that client books All the Wag for. Associated dates can be found via invoice receipts.
    2- Fees: The fees for services are listed on All the Wag’s website: http://www.allthewag.com/services/
    3-Payment: Payment for walks, adventures, and home visits is required in advance. A deposit of half the fee for boarding is due upon booking with the remaining half due on day one of boarding. All the Wag will invoice client through Paypal. Client can pay via, paypal, credit card, or cash.
    4-All the Wag service providers agree to provide all services in a kind, humane, reliable and trustworthy manner.
    5-Medical Care: All the Wag and it’s service providers are authorized to seek any medical care if deemed necessary with release from all liabilities related to transportation, treatment and expenses, and is authorized to approve medical and/or emergency treatment excluding euthanasia as recommended by a veterinarian. Client agrees to reimburse All the Wag for any expenses incurred attending to this need.
    6- Bites: The client is responsible for any costs/payments due to bites. If a bite occurs, All the Wag reserves the right to refuse service and is responsible for reporting it to authorities. All the Wag shall not be held responsible for any damage to Client’s property, or that of others, caused by pet(s) while under the care of an All the Wag service provider.
    7-All the Wag will not be held liable for injury, disappearance, or death of pet(s).
    8-Damages: Client agrees to pay for any and all damage caused to home and/or vehicle and/or property caused by Client’s animal(s).
    9-If keys are needed to enter my house/building, I will provide (2) copies for All the Wag, one for the service provider and one to be kept at All the Wag premises in case of an emergency.
    10-Photographs Occasionally, photographs or video footage may be taken of animals(s) that are using any of the services provided by All the Wag The client gives permission for any resulting photographs or video of Owner’s animals(s) to be used for advertising or educational material, understanding that the resulting photographs or videos are the sole property of All the Wag.
    11-Cancellation: A minimum of 24 hours cancellation notice is required. Cancellations with less than 24 hour notice may be charged at the full rate. For Boarding reservations Seven days notice cancellation is required or the Client’s deposit will not be refunded.
    12- All the Wag reserves the right to terminate this contract at any time before or during this term.
  • INSURANCE

    All the Wag is fully insured by Pet Sitters Associates. All the Wag’s policy covers incidents as long as the dog is kept on leash at all times. If you want us to provide off-leash Buddy Adventures, there is a $500 deductible for all incidents arising from the voluntary releasing of dog(s) in All the Wag’s care, custody or control from its leash. Owner agrees to pay deductible associated with an incident arising during All the Wag Buddy Outings if Owner authorizes All the Wag to release Dog(s) during an All the Wag Buddy Outing.   • RELEASE OF LIABILITY

    I do hereby waive and release All the wag and all it’s service providers from any and all liabilities of any nature for the actions of myself, my pet(s), or any other person who accompanies me, or holds a key to my home; except those arising from negligence or willful misconduct on the part of All the Wag. All the wag agrees to provide all services in kind, humane, reliable and trustworthy manner. Client agrees to notify All the Wag of any concerns within 24 hours of their return. In case of an emergency or a natural disaster I authorize All the Wag to use their reasonable judgment for the care and wellbeing of my pet(s) and/or house. I attest that to my knowledge my pet(s) have not been exposed to rabies or any contagious illness within 30 days prior to beginning its walks/services with All the Wag, and that my pet(s) are current on all legal health and licensing requirements.

    I understand that All the Wag can terminate this contract if my pet becomes a threat to the safety or health of an All the Wag service provider or community due to aggressive behavior. I acknowledge I am responsible for medical expenses and damages resulting from an injury to an All the Wag Service Provider or to any other person or animal caused by my pet(s). All the Wag reserves the right to refuse service to any client, at any time, for any reason. I attest that all of the above information is true to the best of my knowledge and I am the sole decision making owner of pets named above. If anything changes from what is listed, I will inform All the Wag before the next service is scheduled. This signed document gives All the Wag authorization to enter the above listed address as needed to perform the necessary service. I authorize this contract to be valid approval for services so as to permit All the Wag to accept all future telephone, email, mail, on-line reservations and enter my home without additional signed contracts or written authorizations.
